NYS Center for School Health 30 day school physical notice:
NY State Law requires a health examination for all new entrants and students entering Grades Pre-K or K, 1, 3, 5, 7, 9, & 11. The examination must be completed by a NY State licensed physician, physician assistant, or nurse practitioner. NY State also requires a specific form which is available on our website. If a copy is not received within 30 days of the start of school, the school will contact you to determine if you want this physical done by the school physician.
A Dental certificate which states your child has been seen by a dentist or dental hygienist is also asked for at the same time. This form is also available on our website.
You will be contacted prior to any in school physicals occur to have your child scheduled or to resend private physician physical. Please do not send any physicals to school unless one is requested.
The Flu:
NY State Public Health and the Centers for Disease Control and Prevention urge residents to protect themselves against Influenza. Below is a link with key facts on prevention, signs and symptoms to be watchful for, and those at greatest risk. Please consult with your primary physician for recommendations on being vaccinated.
